Referring Physicians

We are happy to accommodate any referrals to the UCSF Epilepsy Center. If you wish to have a patient seen, please contact our schedulers by phone at 415.353.2437.

Alternatively, you may fax a referral to our dedicated Epilepsy Center number at 415.353.2837. Please include all pertinent and recent clinical notes, lab and test results, and radiological imaging data, if possible.

We also utilize a New Patient Profile form, which we ask each patient to complete prior to their visit.

Note: For all Pediatric referrals, please contact Geraldine Dalida at 415.353.8164. Alternatively, fax the patient information and records to 415.353.3573.
